Addy Pacheco was today’s sole witness at the Senate Inquiry. As we have mentioned, Ms. Pacheco featured prominently in the Auditor General’s report as the person who signed off on several applications with invalid documents in some instances as well as incomplete documentation in others. Ms. Pacheco was also the subject of an investigation in 2005 when 200 passports which were issued to her went missing. She was asked about that today at the Senate Hearing.

Mark Lizzaragua, Senator, Senate Select Committee: Can you tell us how these passports that were in your care-

Addy Pacheco, Immigration Clerk: That happened over a weekend. I was not in the office. Someone broke in and stole the passports.

Mark Lizzaragua, Senator, Senate Select Committee: Physically broke the locks?

Addy Pacheco, Immigration Clerk: I can’t say how they got in.

Mark Lizzaragua, Senator, Senate Select Committee: Where did you have them stored?vlcsnap-00378

Addy Pacheco, Immigration Clerk: In the passport office. There was no safe at the time.

Mark Lizzaragua, Senator, Senate Select Committee: Was it in a room under lock and key? Was it in a drawer?

Addy Pacheco, Immigration Clerk: It was in a drawer.

Mark Lizzaragua, Senator, Senate Select Committee: With lock and key?

Addy Pacheco, Immigration Clerk: No.

Mark Lizzaragua, Senator, Senate Select Committee: So anybody that –

Addy Pacheco, Immigration Clerk: Broke into the office would have had access to it.

Mark Lizzaragua, Senator, Senate Select Committee: Or anybody that could have been in the office because it didn’t have a lock?

Addy Pacheco, Immigration Clerk: Yes.

Mark Lizzaragua, Senator, Senate Select Committee: My point is they could have gone missing over a period of time or?

Addy Pacheco, Immigration Clerk: No. It was a specific time because they were delivered, I believe the Friday, and then the Monday they were discovered missing.

Aldo Salazar, Chair, Senate Select Committee: Was there any investigation? Were you questioned about it?

Addy Pacheco, Immigration Clerk: Yes.

Aldo Salazar, Chair, Senate Select Committee: What was the determination? Are you aware as to how the passports went missing?

Addy Pacheco, Immigration Clerk: No.

Aldo Salazar, Chair, Senate Select Committee: But there was a break in into the department? Or they just vanished?vlcsnap-00379

Addy Pacheco, Immigration Clerk: Somebody broke into the building and into the passport office.

Aldo Salazar, Chair, Senate Select Committee: So it was confirmed that there was a break in?

Addy Pacheco, Immigration Clerk: Yes.

Aldo Salazar, Chair, Senate Select Committee: And then the passports were missing over the course of that weekend?

Addy Pacheco, Immigration Clerk: Yes.

Aldo Salazar, Chair, Senate Select Committee: Was there any recovery of any of those passports as far as you’re aware?

Addy Pacheco, Immigration Clerk: I am not sure no.
